He is a mid-tier juggernaut who wins by out-sustaining burst and taking towers faster than anyone. Mayhem's faster gold and constant fighting let him hit his item spikes earlier than on Summoner's Rift. The tradeoff is that he struggles against heavy kiting comps that can kill his Maiden before she reaches them.
Your goal is to summon the Maiden and a full ghoul army, then force the enemy team to fight under their tower. The Maiden applies her mark to champions and structures, letting your ghouls swarm and shred objectives. If the enemy ignores your army to focus you, they take massive damage from your Q and Mist Walkers.
Last-hit dying minions with your basic attacks to spawn graves, then use W to cage a target and raise ghouls with E. In Mayhem's constant brawls, you will usually have graves available near the frontline. Do not wait for a perfect moment—summon what you have and reinforce with new ghouls as enemies die.

Use Snowball to gap-close onto their backline and drop your W on top of them immediately. Your ghouls and Maiden can bodyblock skillshots for you, so summon them before you engage. If you cannot reach their carries, focus on pushing the wave and taking towers while they waste cooldowns on your army.
Champions with large AoE like Ziggs, Syndra, or Miss Fortune can wipe out your ghouls before they reach anyone. Mobile assassins like Zed or Katarina can dash out of your wall and ignore your army to burst you down. Against these comps, play for tower damage and peel for your own carries instead of diving alone.
Look for augments that increase your damage, survivability, or minion stats to make your army more threatening. Anything that gives you healing or shields synergizes with your Q and lets you stay in fights longer. Avoid augments that rely on landing skillshots or stacking ability haste, since your kit already has low cooldowns and point-and-click abilities.
Snowball in, drop your W on their backline, and use E to mark a priority target for your team to focus. You do not have a traditional dash or stun, so your engage is slower than a tank's. Communicate with your team so they follow up when you trap someone, otherwise you will die for nothing.
